Kounis syndrome: a case report of acute myocardial infarction associated with allergic reaction

Kounis syndrome, also known as allergic angina, is a rare but significant clinical condition in which an allergic, hypersensitivity, or anaphylactic reaction precipitates a cardiac event, such as unstable angina or acute myocardial infarction. This syndrome is characterized by the activation of inflammatory cells, mainly mast cells and basophils, which release inflammatory mediators capable of inducing spasm, inflammation, or damage to the coronary arteries. We present the case of a 58-yearold man who arrives at the emergency department with anaphylaxis secondary to the ingestion of sunflower seed bread, with subsequent onset of chest pain. A 12-lead electrocardiogram shows changes in the anteroseptal leads with ST elevation. The patient undergoes percutaneous coronary intervention, confirming a lesion in the left anterior descending artery, for which angioplasty with drug-eluting stent implantation is performed. This report emphasizes the relevance of early diagnosis in cardiovascular emergency situations, underscoring the need for its prompt identification and management.
