Cardiac perforation due to a permanent pacemaker cable is a rare complication but with a high lethality. The clinical picture is variable and depends on the cardiovascular status of the patient, its electrical and hemodynamic stability, and the time of appearance of symptoms; hence its importance in timely identification for subsequent management. This article presents a case of subacute cardiac perforation due to a passive permanent pacemaker electrode, where endovascular extraction was used as a therapeutic alternative.
