Congenital anomalies of the coronary arteries are uncommon, with a worldwide prevalence ranging from 0.2% to 5.8%. These anomalies include anomalous origins, coronary fistulas, and atresia or hypoplasia of the coronary arteries; among them, a single coronary artery is one of the rarest, with a frequency of less than 0.06%. Clinical presentation varies, ranging from asymptomatic cases—occasionally diagnosed incidentally in autopsies or angiographic studies—to acute myocardial ischemia and sudden death. We present the case of a patient treated in the emergency department of a hospital in Northeastern Colombia.
