Heart failure with preserved ejection fraction (HFpEF) accounts for over 70% of heart failure cases in adults aged 65 and older and is associated with high rates of mortality and morbidity. This document provides evidence-based clinical recommendations for the diagnosis and management of HFpEF, tailored to the context of the Peruvian healthcare system. The methodological process followed the GRADE approach, and nine clinically relevant questions were formulated using the PICO framework. The AGREE-II tool was applied to select relevant international clinical practice guidelines. Systematic evidence searches were conducted in international databases, and the recommendations were adapted and contextualized to the Peruvian healthcare setting. Nine clinical questions were addressed, generating a total of three conditional recommendations and 12 good practice points. These recommendations include diagnostic strategies utilizing the H2FPEF and HFA-PEFF algorithms, as well as therapeutic approaches for HFpEF. This clinical practice guideline serves as a key tool to improve the diagnosis and management of HFpEF in Peru, promoting evidence-based interventions aimed at optimizing patient quality of life and reducing complications associated with this condition.
